STREET SMART DRIVING SCHOOL CONTRACT
STREET SMART Driving School agrees to give the student listed below, 30 hours of
Minnesota state-approved classroom training and 6 hours of behind the wheel training for the operation of a motor vehicle in the State of Minnesota for a fee of $375.00.
This agreement constitutes the entire contract between school and student. No verbal statements or promises will be recognized. The school will not refund tuition or part thereof if the school is ready to fulfill its part of the contract. During the behind the wheel lessons, there may be a short period of time when there is another student in the vehicle. In the event of a cancellation, we must have a 24-hour notice prior to the appointment or a fee of $40 will be charged.
Student must complete both classroom and behind the wheel within 16 months from date of contract or contract will be void and incomplete.
The State of Minnesota requires that students verify their identity periodically throughout the course. If the student fails to answer the verification question in time or answers incorrectly, they may be locked out of their course for 24 hours. As this is a state requirement, the student’s course may not be unlocked before 24 hours has elapsed.

You have the right to refuse to supply the data asked for; however, we will not be able to render the services purchased without this data being provided. This data will not be sold to or shared with entities that are not directly involved in providing these services.
