1. It is hereby agreed to give classroom instruction in relation to the operation of
   a motor vehicle for 30 hours.
2. It is hereby agreed to give Behind-the-Wheel instruction in relation to the
   operation of a motor vehicle for 6 hours.
   
3. This agreement constitutes the entire contract between school and student and
   no verbal statement or promises will be recognized.
   
4. In the event of a missed appointment or appointment cancellation, a lesson fee of
   $35.00 per hour, or part thereof, may be charged if 48 hours advance notice is not given.
   
5. The school will not refund tuition or any part thereof if the school is ready and
   able to fulfill its part of the contract.
   
6. This contract may be cancelled at any time by the student. The contract may be
   extended for a maximum of six (6) hours, each extension by signed agreement.
   
7. There may be more than one student in the vehicle.
8. Tennessen Statement: You are hereby warned that the product you are purchasing requires you to supply private or confidential data about yourself. This data will be used to provide you with services you have purchased and to comply with recordkeeping requirements. You have the right to refuse to supply the data asked for, however, we will not be able to render the services purchased without this data being provided. This data will not be sold to or shared with entities that are not directly involved in providing these services.
9. Refund Policy: You may receive a refund for the Headwaters Driving School online course.  There will be a $30 processing fee for all refunds.  Example:  If you paid $150 for the class and would like a refund, the amount refunded to you will be $120.  ($150 - $30 processing fee = $120)
