I agree to the following terms regarding the registration and subsequent completion of this course:

Application and Agreement for Driver Education and Training(As required by the state of Wisconsin Pursuant to provisions of Section 343.71 34 of the Motor Vehicle Code)

I hereby make application to CW Driving School for driver education and training. I agree to pay the fee as follows:

A. Driver education and training program consisting of 6 hours of in car instruction & 6 hrs of observation for a total of $355.00. If payment is made in full.

B. A charge of $54.00 will be made for each in car lesson which is not cancelled 24 hours in advance,showing up without Instruction Permit. A $20.00 fee for signing a new application form when temps and MV3001 form has expired.

C. Refresher in car lessons will be $54.00 per hour. Such instruction will be given only to persons with previous driving experience.

D. The total cost of the ONLINE classroom phase will be $40.00. I will not sign the MV3001 form until the classroom is complete. And OR the entire installment is paid.

E. A not refundable deposit of $135.00 is required when enrolling. With two other payments of $135 to bring the total to $405.00 if paying with installments.

F. It is understood that the trainee may use the driver training vehicle to take their road test for a fee of $225.00 (This includes a 2 hour pre road test lesson, and a driving instructor to take the student to the DMV.

G:This contract is valid for one year, and guaranteed prices for one year. IF this agreement has expired an extension may be filed for $300.00 dollars to complete drivers training. IF gas prices go over $4.00 a gallon an additional $30.00 dollars will be charged and paid before drivers training will be completed.

The driving school is responsible for providing lessons ONLY on agreed time and places.

The driving school WILL NOT refund any tuition if the driving school is able and willing to fulfill its part of the agreement.

This constituted the entire agreement between school and student. and NO verbal statements or promises will be recognized that are NOT in writing on this agreement.

I am under 18 and the total cost for my lessons is printed in A and D .will be $395.00 for online class.
